Medication Refill Request

If your pet is a current patient at WellPet Vets, you can use this form to request a repeat prescription. Please note that, under veterinary regulations, we can only supply medication for pets who have an active veterinary-client-patient relationship. This means your pet must have been recently examined by one of our veterinarians or have sufficient recent health information on file.

If your pet is due for a health check, or if the medication requires ongoing monitoring, one of our team will contact you to arrange a consultation before the refill can be approved. This form is for non-urgent requests only. If your pet is unwell or requires medication urgently, please call the clinic directly.
